Pet name, Gender and the Type . The type is simply dog, cat, horse, bird and so on.

The rest of the data fields are not required but you should use them in order to be as informative about your pet as possible. If the need arises to take your pet to a shelter you can print out this info and give it to the shelter or a temporary care taker. You can have one picture and one movie clip on each pet. As with all forms that have images or movies you can edit the images by hitting the Graphic Editor button or view the movie file by hitting the Play Movie button.
